Pragmatic Free Trial Meta
Pragmatic Free Trail Meta is an open data platform that allows research into pragmatic trials. It gathers and distributes clean trial data, ratings, and evaluations using PRECIS-2. This allows for a variety of meta-epidemiological analyses that compare treatment effect estimates across trials with different levels of pragmatism.
Background
Pragmatic trials provide real-world evidence that can be used to make clinical decisions. The term "pragmatic", however, is not used in a consistent manner and its definition and assessment require clarification. The purpose of pragmatic trials is to guide clinical practice and policy decisions, rather than confirm the validity of a clinical or physiological hypothesis. A pragmatic trial should also aim to be as similar to actual clinical practice as is possible, including its participation of participants, setting up and design, the delivery and implementation of the intervention, determination and analysis of the outcomes, and primary analyses. This is a significant difference from explanatory trials (as described by Schwartz and Lellouch1), which are designed to provide more thorough proof of an idea.
Studies that are truly pragmatic should avoid attempting to blind participants or clinicians, as this may cause bias in estimates of treatment effects. The pragmatic trials also include patients from different healthcare settings to ensure that the outcomes can be compared to the real world.
Furthermore, trials that are pragmatic must concentrate on outcomes that are important to patients, like the quality of life and functional recovery. Many RCTs that don't meet the requirements for pragmatism however, they have characteristics that are in opposition to pragmatism, have been published in journals of varying kinds and incorrectly labeled pragmatic. This can lead to false claims of pragmatism and the term's use should be standardised. The creation of the PRECIS-2 tool, which offers an objective standard for assessing pragmatic features, is a good first step.
Methods
In a pragmatic study it is the intention to inform policy or clinical decisions by demonstrating how an intervention would be integrated into everyday routine care. This is distinct from explanation trials that test hypotheses regarding the causal-effect relationship in idealized situations. In this way, pragmatic trials can have a lower internal validity than explanatory studies and be more prone to biases in their design, analysis, and conduct. Despite their limitations, pragmatic research can be a valuable source of information to make decisions in the context of healthcare.
The PRECIS-2 tool measures the degree of pragmatism within an RCT by assessing it across 9 domains, ranging from 1 (very explanatory) to 5 (very pragmatic). In this study, the recruitment, organisation, flexibility: delivery, flexible adherence and follow-up domains received high scores, but the primary outcome and the method for missing data were below the practical limit. This suggests that it is possible to design a trial with good pragmatic features without damaging the quality of its results.
It is, however, difficult to determine how practical a particular trial really is because pragmaticity is not a definite characteristic; certain aspects of a study can be more pragmatic than others. A trial's pragmatism can be affected by changes to the protocol or the logistics during the trial. In addition 36% of the 89 pragmatic trials discovered by Koppenaal and colleagues were placebo-controlled or conducted prior to licensing, and the majority were single-center. It is therefore important to improve the quality of outcomes ascertainment in these trials, in particular by using national registry databases instead of relying on participants to report adverse events on the trial's own database.
Results
While the definition of pragmatism does not require that all trials are 100 100% pragmatic, there are some advantages to incorporating pragmatic components into clinical trials. These include:
Enhancing sensitivity to issues in the real world which reduces cost and size of the study and allowing the study results to be more quickly transferred into real-world clinical practice (by including patients who are routinely treated). However, pragmatic studies can also have disadvantages. The right type of heterogeneity for instance could help a study extend its findings to different settings or patients. However the wrong kind of heterogeneity can decrease the sensitivity of the test, and therefore decrease the ability of a study to detect minor treatment effects.
A variety of studies have attempted to categorize pragmatic trials using various definitions and scoring methods. Schwartz and Lellouch1 have developed a framework to distinguish between explanation-based trials that support a clinical or physiological hypothesis as well as pragmatic trials that inform the choice of appropriate therapies in real-world clinical practice. The framework was composed of nine domains scored on a 1-5 scale, with 1 being more informative and 5 was more practical. The domains were recruitment and setting, delivery of intervention and follow-up, as well as flexible adherence and primary analysis.
The initial PRECIS tool3 had similar domains and a scale of 1 to 5. Koppenaal and colleagues10 created an adaptation of the assessment, called the Pragmascope, that was easier to use for systematic reviews. This difference in the main analysis domain could be explained by the fact that the majority of pragmatic trials analyse their data in the intention to treat way while some explanation trials do not. The overall score for pragmatic systematic reviews was lower when the domains of organisation, flexible delivery and following-up were combined.
It is important to understand that a pragmatic trial doesn't necessarily mean a low-quality trial, and in fact there is a growing number of clinical trials (as defined by MEDLINE search, however it is neither specific nor sensitive) that use the term "pragmatic" in their abstract or title. These terms may signal an increased understanding of pragmatism in titles and abstracts, but it isn't clear if this is reflected in content.
Conclusions
As appreciation for the value of real-world evidence grows widespread, pragmatic trials have gained momentum in research. They are randomized trials that compare real world care alternatives to clinical trials in development. They involve patient populations more closely resembling those treated in regular medical care. This method has the potential to overcome the limitations of observational studies that are prone to biases associated with reliance on volunteers and limited availability and coding variability in national registry systems.
Other advantages of pragmatic trials include the ability to utilize existing data sources, and a greater chance of detecting meaningful changes than traditional trials. However, they may have some limitations that limit their effectiveness and generalizability. For instance the rates of participation in some trials could be lower than anticipated due to the healthy-volunteer effect as well as incentives to pay or compete for participants from other research studies (e.g. industry trials). The requirement to recruit participants quickly restricts the sample size and impact of many pragmatic trials. In addition, some pragmatic trials do not have controls to ensure that the observed differences are not due to biases in trial conduct.
The authors of the Pragmatic Free Trial Meta identified RCTs published from 2022 to 2022 that self-described as pragmatic. They assessed pragmatism using the PRECIS-2 tool, which consists of the domains eligibility criteria and recruitment criteria, as well as flexibility in intervention adherence, and follow-up.
